  • For pertinent documents that needs to be requested in the registrar’s office, you may call or text 0977 256 1931 or you may alternatively send your concerns to so that we can give you the amount that needs to be paid and the schedule for claiming your document/s.


  • If pertinent documents requested will be claimed by a person other than the document/s owner, an AUTHORIZATION LETTER signed by the document owner and a VALID ID OF THE AUTHORIZED PERSON is needed.
  • As of this writing, we do not send pertinent documents via email for reason of confidentiality.

  • PSA Birth Certificate (3 photocopies)
  • Report Card / Form 138 (original)
  • Certificate of Good Moral – Original
  • 3pcs 2×2 and 1×1 ID Picture
  • 3 long brown envelopes
  • QVR / ESC Certificate (from Private School)

Incoming College

  • Original and 2 photocopies of Form 138
  • NCAE Rating
  • Original and 1 photocopy of Certificate of Good Moral
  • 3 Photocopies of PSA Birth Certificate
  • 4 pieces 1×1 and 2×2 ID picture
  • 3 long brown envelopes
  • Certificate of HRD (only if you are a dependent of an AMCC employee)
