Summer Enrollment Process 2022

Kindly follow these steps for the summer enrollment:

1. FIll up the google form via this link: https://forms.gle/NN4opazJQYiKrLWWA
2. Proceed to the VPSA’s office and present the following documents:
   1. Printed registration form
   2. Accomplished MDR form (Philhealth)
   3. Notarized Parents Informed Consent
   4. Accomplished Leave of Absence/Preference for Learning Continuity
   5. Photocopy of Vaccination Card
3. Proceed to the Cashier (Window 1). Downpayment is Php 3,000.00
4. Proceed to your chairperson’s office and present the registration form together with your receipt for the approval of your subject loading.
5. Proceed to the registrar’s office and present your registration form.
